2/24/2024 EnchantaVaughn Approved 1.Behavior: Josiah greeted QMHP-C as QMHP-C arrived to the home. Josiah appeared to be in a positive mood while discussing with QMHP-C how his day was going. Josiah reported to QMHP- C that his day was good and that he got in trouble by his TDT counselor at which mother reports he will cry uncontrollably with behaviors lasting up to 20 minutes within the school setting. 2. Behavior: Josiah participated in the session with normal behaviors as he did not need any redirection from the QMHP-C to participate in the session. He was praised for his cooperation during the session as QMHP-C continued to encourage him to present positive behaviors on a daily basis. 1.Intervention: QMHP-C reviewed interventions from the previous session and educated Josiah on positive communication skills to utilize while talking with his mother and other authority figures. 2. Intervention: QMHP-C encouraged Josiah to communicate his needs to his mother and to recognize that unless he verbalized them, they may not be addressed. QMHP-C engaged Josiah in a therapeutic activity addressing anger and communication. 1.Response: Josiah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. Josiah was presented with the emotion worksheet to identify his emotion, Josiah shared that he did not recognize an emotion that he could identify with on the sheet and that his emotion was “fine”. 2.Response: Josiah discussed with QMHP-C on how he was going to try to be a better kid because he did not want to keep in trouble with his mother and getting his things taken away from him. He furthered mentioned to QMHP-C on how he continues to try to be good to his brother but his brother does not share with him at home as well as his friends in school being mean to him.
